Medical licensure actions taken

The following medical licensure actions were taken in February and March by the Division of Licensure at the Nebraska Department of Health and Human Services.
Dr. Charles R. James Jr., dentist, Falls City, agreed settlement, censure, $500 civil penalty: Two dental assistants routinely took X-rays without having proper certification.
Dr. Clinton A. Kindler, dentist, Omaha, agreed settlement, censure, $500 civil penalty: Administering nitrous oxide without a permit.
Gerald L. Beckenhauer, funeral director and embalmer, McCook, agreed settlement, censure, civil penalty $1,000: Engaged in misleading advertising by approving ads that offered free information for veterans. The information collected was used in making sales contacts.
R. A. Rozanek, funeral director and embalmer, Fremont, result of a hearing, censure, $2,500 civil penalty, 90-day suspension: Failed to report two DUIs, illegal solicitation of dead bodies. When dead infant twins were transferred to his business by mistake, Rozanek made repeated attempts to persuade the parents to leave them in his care rather than transferring them to a competitor.
Dr. Nancy Taylor Hicks, physician, Omaha, agreed settlement, two years probation: Failed to respond in a timely manner to nurses' notifications of fetal heart problems. Commented upon arrival that she was unhappy about coming in because she was packing for a trip. Child was delivered by Caesarean section with multiple issues, including cardiac arrest and seizure disorder.
Pamela A. Davy, master social worker, Lincoln, agreed settlement, $500 civil penalty, two years probation: Failure to maintain adequate patient records.
William R. Azevedo, mental health practitioner, Omaha, agreed settlement, 60-day suspension, limitations on license forbid him from providing mental health services for temporary staffing or home health care: While providing professional services, subjected patient to third-degree sexual contact and later plead guilty to assault and battery.
Louise K. Hall-Mountain, mental health practitioner, Omaha, agreed settlement, two years probation: Practicing without a license, failure to report termination for alleged unethical conduct.
Julianne Pruitt, licensed practical nurse, Council Bluffs, result of hearing, license revoked: Test was positive for marijuana, dependent on controlled substance.
Linda M. Stout, licensed practical nurse, Saint Joseph, Mo., agreed settlement, $500 civil penalty, one year probation: Failed to administer hydrocodone as officially recorded, failed to report termination.
John M. Furman, registered nurse, Clay Center, agreed settlement, $500 civil penalty, censure, current five-year probation extended one year: Failed to meet drug testing and Alcoholics Anonymous meeting attendance required of probation.
Colleen K. Kapels, registered nurse, Creston, agreed settlement, license revoked: Sleeping while she was the only nurse on duty at a 48-bed long-term care facility.
Kay J. Kube, registered nurse, Crofton, result of hearing, censure, $500 civil penalty, existing three-year probation continued: Failure to report for random body fluids test.
Holly Sue Major, registered nurse, Bellevue, agreed settlement, five years probation: Alcohol dependence.
Lea Anne Crosier, pharmacist, Grand Island, agreed settlement, three months suspension, five years probation: Observed stealing drugs from workplace on police surveillance camera, a search warrant executed on her vehicle yielded 10 drugs, including hydrocodone.
Donald W. Holmquist, provisional alcohol and drug counselor, Lincoln, agreed settlement, four months suspension: Failure to comply with requirements of alcohol treatment program.
Marie A. Keese, dental hygienist, Chadron, agreed settlement, five years probation: Practiced while impaired; alcohol addiction; two DUIs.
Dr. Paulo R. Bahr, physician, Lincoln, result of hearing, three years probation: Prescribed controlled substances, including hydrocodone, to himself; prescribed controlled substances to his family.
Dr. Bruce Howard Marko, physician, Charlotte, N.C., result of hearing, limitations on license: Disciplined in Florida for improper prescribing at weight-loss clinic and failed to pay fine.
Adrienne N. Baumfalk, licensed practical nurse, McCook, agreed settlement, censure, $500 civil penalty: Falsified narcotic count records, failure to report termination.
Laura J. Fuller, licensed practical nurse, Lincoln, agreed settlement, censure: Failure to document physician orders.
Felicia M. Horton, licensed practical nurse, Lincoln, result of hearing, censure, $500 civil penalty: Phoned in renewal of prescriptions, including hydrocodone, without authorization.
Christina L. Ledbetter, licensed practical nurse, Bertrand, agreed settlement, five years probation, 30 days suspension: Admitted dependence on Darvocet, misappropriation of Darvocet.
Heather Rutan, licensed practical nurse, Kearney, agreed settlement, five years probation: Completed treatment for opioid dependency.
Rebecca L. Case, registered nurse, Grand Island, agreed settlement, censure, five-year probation remains in effect: Violation of probation for abuse of alcohol and controlled substances.
Justin T. Peters, registered nurse, Omaha, agreed settlement, censure: Complaining of pain for new dental braces, he checked out Vioxx from a hospital pharmacy for personal use.
Edward M. Reising, registered nurse, Dakota City, result of hearing, 30 day suspension: Failed to report in Nebraska that he was disciplined in Iowa for administering medication to a patient not ordered by a physician.
Linda J. Kuhlenengel, pharmacist, Omaha, unilateral license suspension pending a hearing due to threat of imminent danger: A family member called 911 when she was found unconscious after taking multiple drugs; said the overdose was accidental due to a bad cold.
Jacqueline Okun, alcohol and drug counselor, Omaha, agreed settlement, censure: Willful betrayal of a professional secret.
